Primary Responsibilities:

·         Assist in 5210 Get Up and Move Adams County Initiative.

·         Assist in community events promoting health department services.

·         Assist in All Our Kids (AOK) subcommittee projects and strategic planning.

·         Participate in Substance Abuse Coalition programming and meetings.

·      Complete and submit all necessary IPHA AmeriCorps paperwork and reports by pre-determined deadlines.

·      Attend all IPHA AmeriCorps trainings including orientation and team trainings.

·      Complete all IPHA AmeriCorps required National Days of Service projects.

·      Some local and out of town travel required.

Primary Responsibilities:

·         Plan, organize and conduct individual and group health education, referral programs and activities.

·         Plan, organize, and conduct community health education, outreach, promotion and marketing programs or campaigns.

·         Increase awareness and referrals for Family Case Management and Better Birth Outcomes programs in the community and among medical providers.

·         Assist with community assessments as needed.

·         Provide technical assistance, support and consultation to area providers and agencies to enhance service coordination and program effectiveness for populations served.

·         Participate in community, civic, and professional organizations in the form of meetings, conferences, special courses, and programs.

·         Attend trainings and meetings as needed and required by SCCHD.

·         Complete and submit all necessary IPHA AmeriCorps paperwork and reports by pre-determined deadlines.

·         Attend all IPHA AmeriCorps trainings including orientation and team trainings.

·         Complete all IPHA AmeriCorps required National Days of Service projects.

Primary Responsibilities:

·         Provide nutrition counseling to patients ranging from pediatrics to adults

·         Host workshops on various health topics to build awareness

·         Coordinate and facilitate Patient Support Group Meetings and Cooking Matters workshops

·         Create newsletters and health education bulletin board displays

·         Provide leadership onsite for Reach Out and Read program

·         Assist with appointment adherence and patient reminders

·         Educate new and existing clients on health center programs and resources

·         Assist patients with enrollment in health insurance and prescription assistance programs

·         Assist in community events promoting agency services

·         Assist with agency volunteer coordination and training

·         Participate in health fairs, awareness events, and community partnership and coalition meetings

·         Complete and submit all necessary IPHA AmeriCorps paperwork and reports by pre-determined deadlines.

·         Attend all IPHA AmeriCorps trainings including orientation and team trainings.

·         Complete all IPHA AmeriCorps required National Days of Service projects.
