Primary Responsibilities:

·      Host workshops on various health topics to build awareness in the Evanston and Skokie communities and schools

·      Coordinate and co-facilitate Patient Support Group Meetings and Cooking Matters workshops

·      Assist patients with enrollment in health insurance and prescription assistance programs

·      Assist in community events promoting agency services

·      Assist with agency volunteer coordination and training

Dementia Care Member

Oak Park, IL

The Dementia Care AmeriCorps Specialist will work with the AgeOptions Caregiver Programs team to provide guidance on Dementia issues and support their efforts in the following ways:

Memory Café Duties -

·         Spearhead FY 2021 Memory Cafes and provide support in transition to virtual programming.

·         Collect, manage, and analyze evaluations and reports from AgeOptions memory café sites. Provide technical assistance to Memory Café partners as needed.

·         Support implementation of contracted vendors to provide programming for Memory Café partner sites. This includes, but is not limited to, vendor recruitment, securing contracts, fulfilling reimbursement requests, etc.

·         Maintain database of relevant resources on the AgeOptions Memory Café WordPress Webpage.

ADRD Caregiver Support Duties -

·         Work in partnership with the Caregiver Program Specialist to research and implement strategies around Person Centered support for people with ADRD.

·         Assist partner agencies with virtual and in person programing to include Evidence Based Program interventions, and other training and education programing. The Caregiver Specialist will also be trained in the programs and will conduct classes as needed.

o   Participate in outreach projects to support implementation of Evidence Based Programs in the AgeOptions caregiver network.

·         Support social isolation initiatives as they pertain to those with ADRD.

·         Assist with work with communities to become “Dementia Friendly” Certified.

·         Participate in quarterly meetings with the Caregiver Specialists. Support Caregiver Specialists with technical assistance needs pertaining to virtual programming transition.

·         Maintain a portfolio of updated information regarding ADRD issues and research pertaining to COVID-19. Assess Caregiver Specialists’ need for resources and share information as appropriate.

·         Work with other AgeOptions staff and the Caregiver Specialists in making sure that ADRD caregiver services and programs are culturally competent and meet the needs of diverse groups such as African Americans, Latinos, and the LGBT populations, etc.

·         Other duties as assigned.

AmeriCorps Duties -

·         Serve a total of 1,700 hours over an 11-month period.

Communicable Disease and Emergency Preparedness

Bloomington, IL

·      Participate in scheduled Host Site supervisor meetings.

·      Serve as Point of Dispensing staff member, as assigned, during a public health emergency which requires the dispensing of Strategic National Stockpile medications.

·      Assist with social media messaging development and monitoring during the COVID-19 response.

·      Assist the Communicable Disease program with contact tracing activities.

·      Assist CD Program in developing and implementing an on-site PrEP (pre-exposure prophylaxis) Clinic as part of an IDPH Demonstration Project.

·      Assist with CD Program outreach activities to include: testing events, immunization clinics, and education/information events,

·      Assist with West Nile Virus monitoring activities as requested.

·      Attend emergency preparedness and communicable disease staff meetings.

·      Upload forms/information from previous communicable disease investigations into Epi-Info system and, if time allows, develop training materials using previous communicable disease investigation materials.

·      Assist with vaccination campaigns, communicable disease investigations, and surveillance activities.

·      Support Public Health Emergency Preparedness (PHEP) activities related to the planning and conduct of drills and exercises.

·      Support the development and conduct of MCHD staff PHEP trainings.

·      Support the development and conduct of MCHD Medical Reserve Corps (MRC) volunteer trainings.

·      Support the assessment and revision of MCHD PHEP preparedness and response plans.

·      Develop MRC volunteer recruitment strategies, including social media, identification of outreach opportunities and revisions to the MRC Outreach Toolkit.

Program Coordinator/Assistant

Primary Responsibilities:

1.       Support the implementation and proper documentation of daily programmatic activities: CommunityLinks member encounters (office), database searches, contact attempts (field), and monthly letters.

2.       Complete health risk assessments with located Members and properly document session outcomes in database

a.       Assess Member needs and connect to appropriate MCO supportive services

b.      Process urgent requests for supportive services within 24hrs of completion

c.       Support completion of weekly health risk assessment reports

3.       Support weekly planning and implementation of outreach actives/services   

4.       Support the implementation of weekly monitoring of data quality in database system;

5.       Support planning and co-facilitation of weekly planning meetings

6.       Respond to internal requests for information regarding services provided;

7.       Protect organization's value and manage risk by keeping information confidential;

Primary Responsibilities:

The intent of Independence Center is to provide you with an immersive experience to comprise all of our support departments including Administrative, Psychosocial Rehabilitation, Adult Outpatient Services, Vocational, Housing and Transportation. The majority of tasks will focus on administrative support, but duties will include various other opportunities including case management support, social rehabilitation programming, and vocational correspondence and housing assistance.

Community Liaison

Springfield, IL

Primary Responsibilities:

·      Participate in scheduled Host Site supervisor meetings.

·      To recruit and train 6 public health students at UIS and 2 field coordinators to train and distribute electronic/hard copy information throughout Central Illinois where African-born groups are settled.

·      To identify staff in 8 health departments and 6 FQHCs to become trained in the assessment and care of African individuals with Hepatitis B or practicing risk behaviors leading to Hepatitis B infection.

·      Develop and deliver a series of 4 webinars (2 to health departments and 2 to community-based providers) that focus on Hepatitis B and cultural competency addressing African cultural beliefs and values associated with communicable diseases.

·      Conduct annual virtual meeting with all trainees to facilitate inclusion of Hepatitis B culturally-sound assessment and treatment directed to African immigrants.

·      Develop and create relationships out in the community and other items assigned by site supervisor.
