Population Health - Healthy Futures/Health Education

SIU School of Medicine - Springfield

The AmeriCorps member in this position will assist with work related to Population Health - Healthy Futures/Health Education at Southern Illinois University School of Medicine’s Department of Population Science and Policy in Springfield, Illinois. Through this position, the Member will work with communities to improve health outcomes; promote health education activities in the community; understand health disparities in our service region; assist in conducting research activities; assist in developing data collection tools; compile and analyze data, evaluate, and summarize findings; coordinate the logistics and activities for specific programs; review and summarize literature relevant to current and future projects; assist with community needs assessments; and recruit study volunteers. Emergency Preparedness and Response

Lee County Health Department - Dixon, IL

The AmeriCorps member in this position will focus on emergency preparedness and response at Lee County Health Department through activities to provide assistance to develop volunteer training materials; develop and conduct recruiting presentations; facilitate trainings/meetings; assist in organization and maintenance of documents/records; conduct community education and outreach; emergency preparedness social marketing; volunteer newsletter development; and updating Memorandums of Understanding with community partners. Family Case Management

Tazewell County Health Department - Tremont, IL

The AmeriCorps member in this position will focus on family case management at the Tazewell County Health Department through activities to provide assistance to collect anthromopetric from clients, including height, weight, head circumference, blood pressure and hemoglobin; manage a caseload of assigned clients, providing education and resources; update social history/information of clients seen in clinic; and distribute and review developmental screenings and post partum depression screenings, referring any abnormal results to appropriate staff.  See link below for full position description.

Case management and Aftercare Coordinator

Waukegan, IL

The AmeriCorps member in this position will focus on family case management and aftercare coordination at the The Harbor through case-managing2-3 clients, developing and facilitating New Resident Orientation group as new residents move in, facilitating 2 groups, maintaining intake line and tracking, completing intake screenings and interviews for The Harbor, coordinating and sometimes facilitating monthly Aftercare Program support groups with Bridget or Ray, completing Aftercare Program Outcome Indicator Assessment forms every three months with clients who have graduated from The Harbor, collecting data from the Outcome Indicator Assessments and provide to The Harbor Data Manager as needed, facilitating emergency financial requests from Aftercare clients, attending weekly supervision and attend department and all-staff meetings as scheduled, participating in pertinent training opportunities as available, assist staff with outreach and education for the program as needed, additional duties as assigned and discussed with program clinical manager. See link below for full position description.

Health Education/Promotion

Waukegan, IL

The AmeriCorps member in this position will focus on health education at the Erie Family Health Center: HealthReach Waukegan through activities such as nutrition counseling to students and patients ranging from pediatrics to adults, hosting workshops on various health topics to build awareness, coordinating and facilitating Patient Support Group Meetings and Cooking Matters workshops, creating newsletters and health education bulletin displays, providing leadership onsite for Reach out and Read program, assisting with appointment adherence and patient reminders, assisting patients with enrollment in health insurance and prescription assistance programs, assisting in community events promoting agency service, assisting with agency volunteer coordination and training, participating in health fairs, awareness, and community partnership or coalition meetings. Environmental Health/Outreach

Belleville, IL

·      Plan, organize and conduct community outreach presentations for the Environmental Health programs.
·      Provide assistance with the Smoke Free Illinois, conducting presentations on vaping / e-cigarettes to Junior High and High School students.
·      Provide Assistance with the Food Program, especially with updating contact information, and data verification of the information transferred to the tablet system.
·      Provide assistance with planning, set up and outreach for the annual Earth Day Celebration.
·      Serve a total of 1,700 hours over an 11-month period.
·      Complete and submit all necessary IPHA AmeriCorps paperwork and reports by pre-determined deadlines.
·      Attend all IPHA AmeriCorps trainings including orientation and team trainings. 
·      Complete all IPHA AmeriCorps required National Days of Service projects.
·      Some local and out of town travel required.
