Communicable Disease and Emergency Preparedness

McLean County Health Department - Bloomington, IL

The AmeriCorps member in this position will focus on emergency preparedness and communicable disease at the McLean County Health Department through activities to provide assistance for response to a public health emergency; work in Epi-Info system; on-site PrEP (pre-exposure prophylaxis) Clinic; CD Program outreach; West Nile Virus monitoring; training materials; vaccination campaigns, communicable disease investigations and surveillance; emergency preparedness drills and exercises; MCHD Medical Reserve Corps (MRC) volunteer trainings; MCHD PHEP preparedness and response plans; MRC volunteer recruitment strategies; and community events. Family Case Management

Tazewell County Health Department - Tremont, IL

The AmeriCorps member in this position will focus on family case management at the Tazewell County Health Department through activities to provide assistance to collect anthromopetric from clients, including height, weight, head circumference, blood pressure and hemoglobin; manage a caseload of assigned clients, providing education and resources; update social history/information of clients seen in clinic; and distribute and review developmental screenings and post partum depression screenings, referring any abnormal results to appropriate staff.  See link below for full position description.

Bonaventure House - Chicago, IL

The AmeriCorps member in this position will focus on case management at the Bonaventure House through activities to case-manage 3-5 clients; facilitate new resident orientation; facilitate 2 groups; complete intake screenings and interviews; coordinate and/or facilitate monthly Aftercare Program support groups; complete Aftercare Program Outcome Indicator Assessments with clients who have graduated; collect data from the Outcome Indicator Assessments; facilitate emergency financial requests from Aftercare clients; and assist staff with outreach and education for the program as needed.  See link below for full position description.
