Program Facilitator/Health Education/Promotion

Tuscola, IL

Provide educational outreach related to department programs/services at schools,events,fairs,safety events.
Assist with Summer Food Program promotion, volunteer recruitment, meal site management, personnel scheduling, training of volunteers, food and supply orders, scheduling partnering agencies for daily activity periods, maintaining delivery logs, staff time sheets & mileage, daily meal count sheets, food & supply inventory,  and ordering/pickup/loading/unloading of program supplies & equipment.
Assist with the development of outreach materials, ads, and news releases, presentations, etc in Publisher, Word, Adobe and Excel. Task support as needed for phone calls, eventplanning, fundraising, etc.

·      Process FQHC Medical record request by finding and sending per HIPAA compliant method.

·      Forward any legal or regulatory requests for information to HAH compliance team.

·      Lead the review and response to any participant written grievances.

·      Continuously improve processes to increase efficiency and effectiveness drawing on technology as appropriate.

·      Serve a total of 1,200 hours over a 9-month period.

Epidemiologist/Health Educator

• Establishes scientific and technical methods and practices to identify and evaluate a wide range of health conditions within an assigned program area to recognize public health problems pertinent to the population and determine feasibility of and approach to epidemiologic investigations.

• Performs analyses by using statistical techniques commonly employed in epidemiologic evaluations to interpret and analyze data.

• Assist in writing grants and other funding proposals.

• Identifies surveillance data needs, implements new and revises existing surveillance systems, reports key findings from the system and supports evaluation of surveillance.

• Makes oral presentations to diverse audiences.

• Collect and track data for the strategic plan, and other projects.

• Develop, implement, and sustain a systematic process for assessing customer satisfaction with health department services.

• Work with Public Information Officer in tracking workforce development needs.

• Assist with communicable disease investigations and outbreaks.

• Create a working relationship for data collection and resources.

• Create a resource guide for health department services, as well as needs.

• Assist with outreach efforts for the department as needed.

• Assist with health education as needed.

• Serve a total of 1,200 hours over an 9-month period.

• Complete and submit all necessary IPHA AmeriCorps paperwork and reports by pre-determined deadlines.

• Attend all IPHA AmeriCorps trainings including orientation and team trainings.

• Complete all IPHA AmeriCorps required National Days of Service projects.

• Some local and out of town travel required.
