·         Develop materials to be used in health education sessions provided to employees and community members.

·         Interact with community partners and members to address early childhood learning and development.

·         Partner with coalitions working in the community to improve environmental factors associated with chronic diseases and environmental health hazards.

·         Develop and assist with implementation of social marketing strategies to promote health in the community

·         Promote health equity in the community.

·         Assist victims of domestic violence

·         Make appropriate referrals to county resources

·         Work with staff to help ensure the relevance, efficiency and coordination of legal services for domestic violence victims

·         Complete IDVA training

·         Report suspected child abuse/neglect to DCFS

·         Help to promote PHEP, MRC, and health education programs within communities

·         Recruit MRC/CERT volunteers

·         Give presentations to community groups

·         Develop articles/ads for local newspapers and radio

·         Assist with maintenance of MCPHD Facebook pages

·         Attend local or out of town assigned trainings and meetings

·         Flexible hours are required: some evenings and weekend hours may be required

Primary Responsibilities:

·         Identify and contact clients who are at-risk and/or have become lost to care.

·         Assist clients in scheduling and completing medical appointments; conduct reminder and follow-up calls when appropriate or necessary.

·         Maintain up-to-date, accurate, and culturally competent knowledge of HIV/AIDS prevention, transmission, and treatment.

·         Assist with support groups as needed.

·         Provide direct support to clients during medical appointments, when necessary, which may include new diagnosis support, medication education, and HIV-specific laboratory education.

·         Assist in the development and maintenance of program-level data collection tools, in collaboration with the Linkage to Care team.

·         Assist in data entry and analysis of findings and performance evaluation outcomes.

·         Attend biweekly Linkage to Care team meetings.
